I see this pattern in almost every project that runs late. The clinical evaluation is treated as a document to finalize, not as a process to complete. The difference is not semantic. It determines whether your timeline holds or collapses under the first review cycle.
The reason timelines fail is not lack of effort. It is lack of structure in how the clinical evaluation workstream integrates with the certification path. Most teams work backward from the submission date, not forward from the evidence they actually have. That creates a gap that only becomes visible when it is too late to close it.
The structural problem with most timelines
When you build a timeline for certification, you typically include milestones like technical file completion, risk management update, design verification closure, and CER submission. The clinical evaluation sits somewhere in that list with a placeholder duration. Three months. Four months. Sometimes six.
But that duration assumes the clinical evaluation can start clean and proceed linearly. It assumes the literature search is straightforward, the clinical data is already compiled, the equivalence device is clearly identified, and the appraisal can be written without major gaps. In practice, none of those assumptions hold.
The result is a timeline that looks reasonable on paper but has no resilience when reality diverges. And reality always diverges.
Clinical evaluation timelines are set without assessing the actual state of the clinical evidence. The placeholder duration reflects wishful thinking, not the work required to close the gaps that will emerge during literature appraisal.
What drives the actual duration
The clinical evaluation timeline is not determined by how fast you can write. It is determined by how fast you can resolve the gaps that the process exposes. And those gaps only become visible when you start the appraisal, not before.
You begin the literature search and realize your device classification requires evidence for multiple intended purposes that were never separated in prior studies. You identify an equivalence device and discover it was reclassified in a recent regulatory update. You compile clinical data and find that follow-up intervals do not align with the claims in your IFU. Each of these issues requires decisions, cross-functional input, and sometimes design changes. None of them fit neatly into a Gantt chart.
The clinical evaluation reveals what was not resolved earlier in development. If your device went through multiple design iterations without updating the clinical strategy, you will spend weeks reconciling the evidence base with the current version. If your intended purpose was broadened late in the project, you will need to expand the literature scope and possibly the data collection. These are not delays. They are the actual work.
The dependencies no one tracks
The clinical evaluation depends on inputs from almost every other workstream. Risk management output. Post-market data. Design verification results. Label claims. Each of those inputs must be stable before the clinical evaluation can be finalized. If any of them change, the CER must be updated to reflect the change. That takes time, and it usually happens during the final review cycle when time is least available.
Most timelines do not account for iteration. They assume one draft, one review, one update, and submission. But regulatory reviews generate questions that require going back into the evidence base. Notified Body feedback sometimes challenges the equivalence rationale or asks for additional clinical data. If those requests come two weeks before your planned submission, your timeline has already failed.
The clinical evaluation timeline must include buffer for iteration, not just for drafting. Every review cycle generates findings. Every finding requires resolution. That resolution requires time that most schedules do not allocate.
How to build a timeline that holds
A realistic clinical evaluation timeline starts with a gap assessment, not a target date. You identify what evidence you have, what evidence you need, and what work is required to close the distance. That assessment determines the minimum credible duration. Everything else is negotiation.
The gap assessment must happen before you commit to a certification date. If you set the date first and then discover the gaps, you have already lost control of the timeline. The pressure to meet the deadline will drive shortcuts in the clinical evaluation, and those shortcuts will surface as deficiencies during Notified Body review.
Breaking the work into phases
The clinical evaluation should be structured in phases with defined outputs and decision points. Not milestones. Decision points. Each phase produces something that can be reviewed and either accepted or sent back for revision. If it is sent back, the timeline extends. If it is accepted, the next phase begins.
Phase one is evidence mapping. You define the scope, identify the equivalence strategy, and compile the available data. This phase should produce a clinical evaluation plan that is reviewed and approved by the regulatory and clinical teams. If that plan is not approved, the timeline does not start. You do not proceed to literature search with an unresolved strategy.
Phase two is literature appraisal. You execute the search, screen the studies, and extract the data. This phase should produce a literature review summary that documents the findings and identifies any gaps. That summary is reviewed. If the gaps are acceptable, you move forward. If they are not, you either expand the search or revise the equivalence strategy. That decision determines the rest of the timeline.
Phase three is report drafting. You compile the appraisal, document the conclusions, and prepare the CER. This phase assumes the evidence base is stable. If it is not stable, you are still in phase two. Many teams jump to drafting before the evidence base is closed, and then they spend weeks revising the report every time new data emerges.
The review buffer that no one includes
After drafting, the CER must be reviewed internally before it goes to the Notified Body. That review generates findings. Regulatory will flag inconsistencies with the technical file. Clinical will question unsupported claims. Quality will identify traceability gaps. Each finding requires resolution, and resolution requires time. If you allocate two weeks for internal review, you need at least two more weeks for resolving findings. Most timelines allocate zero.
Then the Notified Body reviews the CER. Their questions require answers, and those answers sometimes require updating the report or providing additional data. If you submit the CER two weeks before your certification target, you have no buffer for that cycle. Any question from the Notified Body will push your timeline past the deadline.
Timelines assume the Notified Body will accept the CER without questions. In practice, every submission generates at least one round of clarification requests. If your timeline has no space for that round, it has already failed.
The decisions that compress timelines
Sometimes the timeline is fixed. The certification date is driven by a commercial commitment, a regulatory deadline, or a market window. In those cases, you cannot extend the duration. You must compress the work. But compression has costs, and those costs must be understood before you commit.
The first decision is scope. If the timeline is too short for a full equivalence demonstration, you may need to narrow the intended purpose or reduce the claim set. That is not ideal, but it is better than submitting a weak CER that will be rejected. A narrower scope that is well supported is preferable to a broad scope that is poorly justified.
The second decision is resource allocation. If you need to complete the clinical evaluation faster, you need more people working on it. That means bringing in external support, dedicating internal clinical resources full-time, or splitting the workstream across multiple writers. None of those options are free, and they all require coordination overhead. But they are the only way to compress the timeline without sacrificing quality.
The third decision is iteration tolerance. If you compress the timeline, you cannot afford multiple revision cycles. That means the first draft must be closer to final quality. That requires more upfront planning, more detailed clinical evaluation plans, and more senior involvement earlier in the process. Junior team members can execute the literature search, but they cannot make the strategic decisions that determine whether the evidence base is sufficient.
What you cannot compress
Some parts of the clinical evaluation cannot be compressed. Notified Body review takes the time it takes. Literature database access has fixed processing times. Internal regulatory review requires sufficient time to read and evaluate the report properly. If you compress those elements, you introduce risk. And that risk will materialize as deficiencies, rejections, or delays that are worse than the original timeline pressure.
I have seen teams submit a CER knowing it was incomplete because the deadline could not move. The Notified Body rejected it. The resubmission took another three months. The total delay was longer than if they had extended the original timeline by six weeks and submitted a complete report the first time. The pressure to meet the deadline created the delay it was meant to avoid.
Compressing the clinical evaluation timeline is possible, but only if you make deliberate tradeoffs on scope, resources, or claims. Compressing it by simply working faster or skipping review cycles does not work. It only shifts the delay to a later stage where it is harder to recover.
The timeline conversation that should happen earlier
The conversation about clinical evaluation timelines should happen during design planning, not during certification planning. By the time you are scheduling the Notified Body submission, the clinical strategy should already be executed. The CER should be in maintenance mode, not initial drafting mode.
But most organizations treat clinical evaluation as a certification activity rather than a development activity. It starts when the design is locked and the technical file is being compiled. At that point, any gaps in the clinical evidence are already embedded in the timeline. You cannot generate new clinical data quickly. You cannot redo a literature search that should have been done a year earlier. You can only work with what you have, and hope it is sufficient.
The better approach is to integrate the clinical evaluation workstream into the development process from the beginning. Conduct the literature review during design input definition. Identify the equivalence device before finalizing the design. Collect clinical data during verification and validation, not after. By the time you reach certification planning, the CER should be a matter of updating and finalizing, not creating from scratch.
That requires a shift in how organizations think about clinical evaluation. It is not a document. It is a process that runs parallel to development. And like any process, it requires planning, resourcing, and management. If you treat it as an afterthought, your timeline will reflect that.
The reality check before committing
Before you commit to a clinical evaluation timeline, answer three questions. First, what evidence do we have today. Not what we expect to have. What we have. Second, what evidence do we need to support the claims in the IFU. Not what would be nice to have. What we need. Third, what work is required to close the gap between those two. Not what we hope will work. What we know will work.
If you cannot answer those questions, your timeline is not based on reality. It is based on optimism. And optimism is not a strategy for meeting regulatory deadlines.
The clinical evaluation timeline that works is the one built from evidence, not from hope. It accounts for iteration, includes buffer for review cycles, and integrates with the certification path without assuming everything will go perfectly. It is not the shortest possible timeline. It is the shortest credible timeline. And that distinction is what separates the projects that meet their deadlines from the ones that do not.
